Yes ... will attach asap, in the meantime a small version can be seen at existing website in upper left corner
1d) Do you have a corporate visual identity (a customized (non stock) logo and color scheme carried out throughout your office, stationery, business cards, etc.)?
Generally we've done a blue or blue/grey theme, but we're definitely not married to that.
1e) What is the nature of your business?: Briefly describe your products or services:
Eye doctor, top quality medical care plus the retail side of glasses and sunglasses. The retail side carries both the every day types of glasses one would expect at their eye doctor, but also carries a significant high end stock of both eye glasses and sunglasses.
1f) Who is your target market? Who are you hoping to reach with your site?
The goal is two fold... one, we're in a growing area, and we want to reach new residents. Two, we want to establish some basic communication between our clients and ourselves by adding some simple web forms in the future. The target market is hugely varied, we both need to look sexy and hip for the young professional crowd that we service and need to be simple enough to navigate so our non-tech savvy clients can still find the useful information.

No, not yet, though we'll add some of our own later. I'd imagine photography of faces wearing glasses, or simple eye shots, stock photos of people putting in contact lenses, eye charts, frames on nice backgrounds.

Yes
3c) If so, will you require it to be ONLY CSS layout? (Yes/No --- {fees})
Not necessarily, but we do plan on being able to navigate the site using a screen reader, so a heavily table based layout is not acceptable.
4c) How many pages coded will you require delivered? ({fees})
I imagine 2. A home page, then a relatively customizable page (move photos from right column to left for example) for the interior pages. I wouldn't be surprised if you talked me into a 3rd page to help add some variety, for this I would imagine 2 interior pages, one which is primarily photo based and one which is primarily text based.
Are there any other comments/specific ideas we should keep in mind while designing your project?
I'm a total prude for consistent navigation, I despise going from a home page to an interior page and having my navigation flipped on me. Given the long term plans for the website there will likely be a significant number of navigation links, so room to grow there would be a big plus.
