I need an opt-in page, fully coded and ready to go, based on the info here:

I made this page for your info and to have simple access to these graphics.

The copy and headline are fine, but leaving some more room for a few more bullets is good. The large graphic on top can be used or resized to use as you see fit.

I have also stuck on a graphic header you may use. It has colors consistent with the existing site.

http://www.profitmart.com/ is a reasonable competitor site.


5b) What type of look are you trying to achieve? (e.g. clean, high-tech, conservative, warm, flashy, etc.)

Looking for something very slick and professional. I can do basic html, I want it to look inviting and slick.
I want the opt-in submit button to be a graphic with the same words on the existing submit button. This button should be a 3-D thing I'm just dying to click :)


6b) Please provide the addresses of competitors' sites, if possible:

http://www.profitmart.com/ is a reasonable competitor site. In terms of graphic professionalism it is good. Don't like the color scheme.


7b) Do you have any images for use in the design of your site? If not, do you have images, can you offer suggestions of what you type of imagery you feel would be most suitable for your website:
