Copyright Form?

On one of the logo contests, there was mention of the designer completing and e-mailing a copyright form for the finalized design. What is this all about? Is this form necessary for copyright purposes? Should this be provided by the designer in all contests?

If you plan on copyrighting your logo you should have the designer fill out the form linked from this part of the help section...

It is provided if you ask for it and is only for logos. Illustrations have a separate copyright agreement. There is no copyright transfer for any other types of design work.
