Hi,
This contest is different than the other ones I have posted.
This time I am not looking for the best-looking site. What I want is a site that will "convert" well. Meaning... The point of this site is to get people to fill out a form so that they can be contacted. the more people fill out the form the better!
The site we are redesigning is:

this site is well listed on major search engines for the term "Mesothelioma" - a highly competitive term for search engine marketers. The problem is, we feel sure a better design will help us get more people to fill in the forms or call the 800 number..
What we need:
- Two page HTML template that keeps the same text and headings as current.
- CSS and graphics files - including original graphics - so we can make changes at later date.
- Template design that instills confidence in the site as a reliable website. (Blue and Green colors are reputed to do this)
- Form needs to be above the fold.
- Should include a clear call to action - "To get a free mesothelioma information pack sign up now!"
- Should also feature prominently the 800 number to call for more information.
- Can include a button or buttons telling user to click to sign up now - which will pop up a small window containing the same form.
- graphical element should be a middle aged all-American couple or family - working people. They should look like the kind of people who worked hard their whole life and want to enjoy the fruits of their labor - as this is the typical mesothelioma sufferer's profile.
- The medical symbol that is on the site now can be used or removed per your choice.
- The site should fit a 800X600 Screen and be centered for those with larger screens.
- While I usually like very curvy headers with soft colors - I think that a site that should be authoritative and trustworthy may work better if it is more formal and boxy. That said - you are the designers and we will be happy to see your creations either way!
- Important - much of the the target audience is likely to be on dial-up connections - graphics need to be such that they load quickly on a dial-up connection.
We need initial designs by Thursday March 24th! Winner or finalists will be chosen soon thereafter.
I will probably be showing the best designs to large group of regular internet users to get their opinion on which one would be more likely to make them call or fill in a form.
